Description of Services

The Homelessness Crisis Response Program provides temporary rental assistance for individuals or families experiencing a momentary emergency that may jeopardize their housing. The program was designed to assist individuals and families who are experiencing homelessness, or at imminent risk of literal homelessness, obtain permanent housing. Case managers meet with and assess all participants and their needs. In addition to rental assistance, the team will make all applicable referrals to other agencies and programs that can assist the consumer in continuing their self-sufficiency.

Appropriate Referrals to Homelessness Prevention: Low-income individuals or families currently experiencing homelessness.

Eligibility Requirements: Referral from Homeless Shelter.

The Supportive Housing Program is designed to promote the development of self-sufficient individuals and families by offering supportive services as well as rent subsidies to assist homeless persons in transitioning from homelessness to independent living. To be eligible for assistance under this program, the head of household must be considered homeless by HUD standards (i.e., sleeping in places not meant for human habitation such as cars, parks, sidewalks and abandoned buildings or sleeping in an emergency shelter as a primary nighttime residence) and have a verifiable disability that is expected to be of long duration.

Referral from shelter or homeless outreach worker.
